We appreciate your interest in serving in MV Kids.

We know that this is a very personal and lengthy application for you to fill out, but please be assured that these precautions are necessary in order to provide the safest environment for our children we possibly can.

Information contained in this application will be disclosed only to those who have a genuine need to know in order to carry out their responsibilities at MVC. As a follow-up to this application, a Live Scan (fingerprinting and background check) will be required.
